Note Regarding Providers
Although provider directories are updated regularly, it is important that you verify with your doctor that he/she will be participating in the health care plan. If your doctor discontinues participating in the plan mid-year, your option for the remainder of the benefit year will be to select another doctor within the network or to continue to see your doctor on an out-of-network basis resulting in higher out of pocket costs to you.
